Form Information

Centres should be aware of learner registration expiry dates for different qualifications and ensure certificates are claimed in a timley manner, however we recognise sometimes a learner may need an extension to enable them to complete their qualification.

Please read the Learner registration Extension request policy (accessible via QMIS) before completing this form.

All requests will be reviewed and you will be informed of our decision.

If applicable, please submit a list upload. Please ensure you include the following information: learner names, learner numbers, registration date, qualification name, % progress made to date, length of extension requested, learners completion date and the reason for the extension.

I confirm that the Centre’s Senior Management Team supports this application to extend the learners registration.
I confirm that the Centre understands that any extension is not guaranteed until FutureQuals have confirmed so in writing.
I can confirm that the Centre understand that extension requests must be submitted well in advance of the learner's original registration expiring.
I can confirm that the Centre understand that if the learner's registration has expired then a full registration fee can be charged by FutureQuals.
