This form should be used to report any suspected or actual malpractice or maladministration involving a FutureQuals Approved Centre, learner, assessor, or any other individual involved in the assessment, quality assurance, design, delivery or award of a FutureQuals qualification. Please provide all relevant information known at the time of submission.

Qualifications Regulated by Qualification Scotland Accreditation.

If the notification received relates to a qualification regulated by Qualification Scotland Accreditation, in accordance with Regulatory Principle 18, FutureQuals “must inform Qualification Scotland Accreditation at the earliest opportunity, preferably prior to investigatory activities, of suspected or actual cases of malpractice and/or maladministration."
